CHRONIC PYELONEPHRITIS AND PREGNANCY: IMPACT OF COMORBIDITY AND ITS MANIFESTATIONS

Abstract

The World Health Organization classifies chronic pyelonephritis (CP) as a problem of social importance, since the disease usually develops in people of working age and often causes disability in patients. The prevalence of CP varies from 8 to 20%, according to the causes of death. One of the urgent problems of modern obstetrics is combined somatic pathology, which has a negative impact on the reproductive function of a woman, complicates the course of pregnancy and childbirth, which is manifested by the development of hypertensive disorders of pregnancy, miscarriage, the birth of immature children, an increased frequency of postpartum renal and extrarenal complications, leading to antenatal death of the fetus, complications from the mother. The study analyzed the condition of 53 pregnant women with chronic pyelonephritis admitted to the obstetric department of the multidisciplinary clinic of the Samara State Medical University for the periods of 2021-2023, divided into 2 groups: the first group included 35 pregnant women with exacerbation of CP, the second group included 18 pregnant women without exacerbation of CP . Research methods - general clinical examination methods (collection of complaints, anamnesis, objective examination), laboratory examination methods (OAC, OAM, Zimnitsky test, Nechiporenko test, blood biochemistry, coagulogram, bacterial urine culture), instrumental examination methods (ECG, ultrasound, echocardiography) . All data were statistically processed in the Statistic 6.0 program. The study revealed complications of pregnancy and a violation of the somatic health of a woman in the form of disorders in the function of the cardiovascular system, which was reflected in the form of hypoxia, ventricular hypertrophy, impaired conduction of the heart and impaired pumping function of the heart. It can be concluded that comorbidity occurs in pregnant women with chronic pyelonephritis, and mutually aggravates the course of pregnancy.
