Abstract

Relevance. Primary open-angle glaucoma (POAG) is a multifactorial neurodegenerative disease characterized by accelerated optic neuropathy, which remains one of the main causes of visual impairment. Purpose of the study. To evaluate the effectiveness of a comprehensive approach to the treatment of patients with POAG, taking into account the identified disorders of retinal microcirculation and the reactivity of vascular endothelial dysfunction. Material and methods. The study involved 94 POAG patients aged 65 to 80 years. Results and conclusion. The results of this section of the work showed a decrease in the density of blood flow and vessels already at the initial stage of the disease. The data obtained suggest that a decrease in blood flow in the optic nerve head and peripapillary retina, detected by the OCT-A method, can be determined earlier than functional and structural losses determined by the OCT and SAP methods. These data open up new prospects for the use of OCT-A in glaucoma diagnostics.
