MIGRATION OF A METALLIC KIRSCHNER WIRE AFTER CLAVICULAR OSTEOSYNTHESIS INTO THE RIGHT LUNG: A CLINICAL CASE

Abstract

This study aims to present the diagnostic features and surgical management of delayed migration of a metallic Kirschner wire following clavicular osteosynthesis into the right lung. A descriptive analysis of a single clinical case was conducted based on preoperative and intraoperative data. A 63-year-old male with a history of clavicular fixation 15 years prior presented with right-sided chest pain. Computed tomography revealed a metallic foreign body in the right hemithorax. Surgical exploration via thoracotomy identified intrapulmonary localization of the wire, dense pleural adhesions, and a mediastinal mass, complicated by bleeding from the azygos vein. The foreign body was successfully removed, and hemostasis was achieved. The case demonstrates that delayed migration may remain clinically silent or present with nonspecific symptoms. Computed tomography is essential for diagnosis, although the full extent of pathology may be revealed intraoperatively. Timely surgical intervention in specialized thoracic settings is critical for effective management and prevention of severe complications.
