Abstract

Executive decision-making in regulated innovation environments demands a nuanced convergence of strategic leadership, regulatory intelligence, and organisational adaptability. This chapter examines how pharmaceutical and medical device executives navigate complex regulatory frameworks—including FDA pathways, EU MDR, and evolving AI-specific guidelines—to drive sustainable innovation. Drawing on recent empirical and conceptual literature, the chapter presents a framework integrating top management team competencies, risk governance, and digital transformation imperatives. . Key themes include the role of regulatory affairs in shaping pipeline strategy, leadership capabilities required for Industry 4.0 adoption, and the organisational enablers of innovation performance. The chapter contributes actionable insights for executives, policymakers, and scholars concerned with optimising the intersection of human capital, compliance, and value-creation in highly regulated industries.
