P4‐278: Pharmaco‐economic aspects of senile dementia

The comprehensive biopsychosocial approach in the form of targeted psychosocial work on the background of rational antidemential therapy has scientifically proven resource saving potential. In this case inadequate therapy leads to a waste of limited health care costs. The aim was to study of the effect of pharmacological costs on the quality of the economic burden of medical care for patients with senile dementia. We studied 54 patients suffering from senile dementia with episodes of prescribing psychotropic drugs for the period from 2013 to 2015 in the Tashkent Medical Academy. The cost of medical care was calculated on the basis of the approved price lists. Up to 30% of the monitored patients were over than 60 years (the ratio of women to men 2-2.5: 1). Patients usually with accompanying, addressed to doctors 5,7 ± 1,8 (1-8) times a year, in 95% of cases in connection with a prescribing medication. Calls a doctor at home accounted for less than 5%.The proportion of treatment in a day hospital - less than 10%; the average duration of treatment episode - 23.5 days. Memantine is recommended for the treatment of advanced stages of AD and vascular dementia as the drug of first line shown in connection with clinical and economic benefit. Currently, acetylcholinesterase inhibitors are regarded as agents with the most evidence base in terms of correction of cognitive impairment in Alzheimer's disease. This group of drugs recommended for the treatment of light and moderate stages of the disease. In the early stages of the disease acetylcholinesterase inhibitors may have a pathogenetic effect, restoring the central characteristic for AD acetylcholinergic defect. Acetylcholinesterase inhibitors were prescribed less than 20% of patients; 3/4 of them received them no more than three consecutive months. For improving cognitive functions a wide range of neuroprotective and vasoactive drugs, agents with antioxidant, and the alleged neyrometabolicand neurotrophic effect, applied. 50% of patients were taking cerebroprotectors and proofreaders of microcirculation usually no more than three consecutive months. Evidence-based selection and the systematic application of antidementialdrugs can relieve the burden of dementia for the family of the patient, neurological and psychiatric health services and society in general.
