<b>Goal:</b> Study of heart rate variability in patients with chronic obstructive pulmonary disease (COPD) complicated by pulmonary heart disease. <b>Material and Methods:</b> 88 patients with chronic obstructive pulmonary disease (COPD) of the III-IV degree were examined. Heart rate variability (HRV) was assessed and pulmonary hypertension and dilation of the right ventricle were determined. <b>Results:</b> Analysis of the spectral parameters of HRV in the group of patients with pulmonary hypertension showed an increase in the low frequency index (LF), reflecting sympathetic effects on the heart, a decrease in the high frequency index (HF), characterizing the influence of the vagus nerve, respectively, and an increase in the LF / HF ratio. In the group of patients with drugs with dilatation of the right ventricle, an increase in the revealed changes was noted; there was a decrease in the high frequencies by 26.7% (p &lt;0.01), an increase in the low frequencies by 13.7% (0.05) and the LF / HF coefficient by 18.6% (p &lt;0.05). As drugs progressed, the average heart rate increased from 86.1 ± 0.9 beats per minute to 93.5 ± 0.8 beats per minute, and the maximum and minimum heart rates similarly changed. <b>Conclusion:</b> Changes in the spectral and temporal parameters of HRV indicate a pronounced autonomic imbalance that increases in patients with dilatation of the right ventricle of the heart and pulmonary hypertension.
