Abstract: The article presents the results of studying extracts from the case histories of patients with a diagnosis of coronary pulmonary disease. Angina pectoris. FC II" and "Type 2 diabetes mellitus". An extract from the case histories is necessary for doctors of outpatient clinics and is a key document for improving the continuity in the organization of medical care and improving their quality. It was revealed that mainly in the paragraph of recommendations attention is paid to drug therapy, which, of course, is important. At the same time, outpatient clinic doctors require more detailed information on physical activity, nutrition, disease prevention, etc.
