Аннотация

Background: Streptococcus pneumoniae is one of the major causative pathogens of COPD. Studying of regional gene-serotype specificity of S. pneumoniae is necessary to assess the effectiveness of pneumococcal vaccines in patients with COPD. Purpose: to investigate regional geno-serotypic specificity of s. pneumoniae in patients with chronic obstructive pulmonary disease during a respiratory exacerbation. Materials and methods: In 96 patients, pathological material was taken from the nasopharyngeal mucosa. S. pneumoniae was determined by multiplex PCR, which was carried out in several stages. At the initial stage, the presence of virulence/pili genes was determined, after which multiplex reactions with primers were performed to identify serotypes and serogroups. Results: Frequency of occurrence of s. pneumoniae in cohort of the surveyed patients was 44.8%, among COPD patients - 38.7% (24), BA - 55.9% (19). In patients with BA was noted the occurrence of lytA genotype (42%) with predominant frequency of serotypes of 5(26,3), 22F/22A (21,5%) and 19F (15,8%). In patients with COPD the occurrence of lytA genotypes (41,7%), cpsA (37,5%) and combination of cpsA and lytA (20,8%) noted with predominance of serotypes 11A/11D (20,8%), 19F (20,8%) and 9L/N (16,7%). Conclusions: In cohort of patients, COPD was noted with both genotypic and serotypic specificity of s. pneumoniae, determined by compatibility of cpsA and lytA genotypes and serotypes 11A/11D and 9L/N.
