Resume. Spontaneous bacterial peritonitis (SBP) represents a life-threatening complication of decompensated liver cirrhosis, with viral hepatitis-induced cirrhosis constituting a predominant etiological category globally. This comprehensive review examines the pathophysiological basis, diagnostic criteria, antimicrobial therapeutic strategies, and preventive measures for SBP in patients with viral etiology cirrhosis. Particular attention is devoted to the distinctive clinical features of hepatitis B virus (HBV), hepatitis C virus (HCV), and hepatitis D virus (HDV) associated cirrhosis, evolving antimicrobial resistance patterns, and the integration of antiviral therapy with SBP management. Evidence-based recommendations for empirical antibiotic selection, treatment duration, secondary prophylaxis, and assessment of liver transplantation candidacy are synthesized from contemporary literature.
