We propose a simple mechanism, anchored in weak-coupling BCS theory, which ties together the following facts: high ${T}_{c}$; quasi two dimensionality; orthorhombic distortion and/or disordered lines of oxygen; proximity to a metal-insulator transition; and anomalously small isotope effects.
