Abstract. At present, there are several markers, including Ki-67, applied to make a prognosis for tumor behavior to identify its aggressiveness. Our study aims to examine Ki-67 expression in somatotroph adenomas with particular emphasis on evaluation of long-term outcome and correlation between Ki-67 and other tumor activity parameters. We studied 29 patients treated with acromegaly (May 2016 – December 2019). All patients were diagnosed with acromegaly and treated surgically. Ki-67 level in men was within the safe limits, while women showed high mitotic index of Ki-67. Positive correlation was discovered between Ki-67 level and remnant tissue and pre-op GH, and pre-op IGF-1. Overall, Ki-67 expression in somatotroph tumor varied from 0.1% to 12% with mean 2.7±0.5%. 41.4% of patients had Ki-67 >3%, which is negative prognostic factor for disease outcome
