Both Phage and vaccine Prevenar-13 prevent patients with COPD form community acquired pneumonia

<bold>Background.</bold> Patients with COPD are at high risk of community acquired pneumonia (CAP). <bold>Aim:</bold> to assess effect of Phage and vaccine Prevenar-13 (P13) in patients with COPD in preventing of CAP. <bold>Methods.</bold> 102 patients with COPD were examined. 30 received P13 ones sc. 29 were prescribed Str Phage via nebulizer 2ml tid during 10 days. 43 patients who did not use vaccine or Phage were included into the control group. Frequency of exacerbations, acute respiratory infections (ARS), need for antibacterial therapy, and hospitalizations during the year were assessed before and 1 year after treatment. <bold>Results:</bold> number of COPD exacerbations decreased in P13 (4.75±0.27 to 2.58±0.33 per year, p<0.001) and Phage group (5.07±0.22 to 2.54±0.45, p<0.0001) both significantly comparing to the control (4.2±0.25; p<0.005). Frequency of ARS decreased in P13 from 3.93±0.16 per year (p<0.001) and Phage from 4.15±0.14 to 1.85±0.23 (p<0.001) and both comparing to the control (p<0.001). Need for antibacterial therapy decreased in P13 group from 2.79±0.15 times per year to 1.31±0.18 (p<0.0001) and in Phage group 2.85±0.101 to 0.69±0.14 (p<0.001) both comparing to the control (1.87±0.13; p=0.014 for P13 and p<0.001 for Phage). Number of hospitalizations decreased with p<0.0001 in P13 (2.07±0.07 to 0.76±0.11) and Phage group (1.92±0.05 to 0.46±0.09), both comparing to the control (1.33±0.11, p<0.001). Phage group had lower need for antibacterial therapy (p=0.008) and hospitalizations (p=0.04) comparing to P13. <bold>Conclusion:</bold> Both Phage and vaccine P13 prevent patients with COPD form CAP decreasing frequency of exacerbations, ARS, need for antibacterial therapy, and hospitalizations during the year.
