Abstract Background and Aims Chronic kidney disease (CKD) represents a significant healthcare challenge in Uzbekistan, with an estimated prevalence of 13.1% among the adult population. Despite recent improvements in healthcare infrastructure, access to quality hemodialysis remains limited, particularly in rural areas. This study aims to evaluate the current state of hemodialysis services in Uzbekistan and propose evidence-based strategies for improving care delivery and patient outcomes. Method We conducted a mixed-methods study between January 2023 and December 2023, incorporating both quantitative and qualitative approaches. Data was collected from 28 dialysis centers across 12 regions of Uzbekistan. We analyzed medical records of 1,245 patients receiving maintenance hemodialysis and conducted structured interviews with 156 healthcare providers. Key parameters evaluated included: Statistical analysis was performed using SPSS version 26.0, with P < 0.05 considered statistically significant. Results Analysis revealed significant disparities in hemodialysis care access and delivery: Regional analysis demonstrated that only 35% of dialysis centers met international standards for water quality management and infection control protocols. Conclusion Our findings highlight substantial gaps in hemodialysis care delivery in Uzbekistan, particularly in rural regions. We recommend implementing a comprehensive national program focusing on: These interventions could significantly improve patient outcomes and align Uzbekistan's dialysis care with international standards.
