The objective is to evaluate the prognostic value of serial resistive index (RI) monitoring for the early detection of complications after pediatric kidney transplantation. The material and methods of the study are pilot single-center retrospective-prospective cohort study enrolled 31 children aged 6–18 years who underwent living-related donor kidney transplantation between 2023 and 2025. RI and acceleration time (AT) were measured at nine serial Doppler time points. The results showed that an unfavorable composite outcome occurred in 5/31 patients (16.1%). The resistive index on postoperative day 3 was significantly higher in the unfavorable outcome group than in the favorable group (0.77 vs 0.70, p < 0.05). One patient demonstrated a parvus-tardus pattern (AT increased from 69 to 118 and then to 129 ms with preserved RI), confirmed as transplant artery stenosis. Conclusions: Day-3 RI is the most informative early predictor of unfavorable outcome in pediatric kidney transplantation. Serial Doppler monitoring should be incorporated into standard post-transplant care protocols.
