INTEGRATED SURGICAL APPROACH TO PATIENTS WITH VENTAL HERNIATION AND OBESITY

<strong>The aim of the study </strong>is to improve the quality of treatment of ventral hernias in patients with obesity by improving the tactical and technical aspects and optimizing the choice of hernioplasty method. <strong>Materials and research methods. </strong>The study is based on a clinical and laboratory examination of 121 patients with ventral hernias who were operated on in the surgical department of the 1st clinic of the Samarkand State Medical University for the period from 2012 to 2021. <strong>Research results. </strong>The use of an improved pneumatic bandage allowed to significantly improve the quality of preoperative preparation of patients with ventral hernia and morbid obesity, reduce the risk of developing intra-abdominal hypertension after hernioplasty, achieve an improvement in the patient's condition at an earlier date and, accordingly, reduce the incidence of postoperative extra- abdominal complications from 7.4% to 4.5% . <strong>Conclusions. </strong>The application of the proposed algorithm, taking into account the improved tactical and technical aspects of surgical treatment of ventral hernias in patients with morbid obesity, made it possible to reduce the overall incidence of postoperative complications from 14.8% to 8.9% (p=0.045), including wound complications from 11.1% to 4.5%, as well as significantly reduce the duration of surgical treatment and rehabilitation periods after various types of hernioplasty and abdominoplasty.
