Resume. Objective of the study. To conduct a systematic analysis of the data available in the current literature regarding the recurrence rate after radical trachelectomy in patients with invasive cervical cancer, the timing of recurrence, the locations, treatment options, and to assess the risk factors for recurrence depending on the surgical access approach. Materials and Methods. The review includes data from foreign and domestic articles found in PubMed on this topic, published over the last 5 years. Results. Radical trachelectomy is the standard surgical treatment for early-stage invasive cervical cancer in young women who wish to preserve reproductive function. The oncological safety of radical trachelectomy has been proven in many studies, and clear criteria for patient selection for radical trachelectomy via various approaches (vaginal, abdominal, laparoscopic, robot-assisted) have been developed. However, in some patients, disease recurrence occurs. Since this surgery is performed infrequently in various oncological institutions and the accumulation of data is slow, the analysis of treatment results in small numbers of patients often does not allow authors to make definitive conclusions. The current analysis includes over 40 studies. Conclusion. The data obtained from the analysis confirm the need for a strict selection of patients with invasive cervical cancer for organ-preserving treatment to achieve optimal oncological outcomes. Keywords: organ-preserving treatment for cervical cancer, radical trachelectomy, oncological outcomes, recurrences.
