Ileal prolapse through a patent omphalomesenteric duct associated with multiple congenital malformations: A case report

Patent vitellointestinal duct (PVID) is a rare anomaly of the omphalomesenteric duct that may present with umbilical discharge and, in rare cases, ileal prolapse. We report a 4-month-old male infant who presented with erythema, bleeding, and passage of stool and gas through the umbilicus. The patient had multiple associated congenital anomalies, including congenital heart disease (patent ductus arteriosus and atrial septal defect), previously repaired cervicothoracic meningomyelocele, and right-sided polycystic kidney disease. Imaging studies confirmed a patent vitellointestinal duct complicated by ileal prolapse. The infant underwent a subumbilical laparotomy with reduction of the prolapsed bowel, wedge excision of the patent duct, and primary ileal repair. The postoperative course was uneventful. The presence of an intestinal prolapse through the umbilicus should raise suspicion for a patent vitellointestinal duct.
