Leiomyosarcoma of Paratesticular Region: A Rare Encounter

Leiomyosarcoma (LMS) of the Para testicular region is an exceptionally rare malignant tumour arising from smooth muscle elements of structures such as the spermatic cord, epididymis, and testicular tunics. We report the case of a 49- year-old male presenting with an 18-month history of painless right testicular swelling. Clinical evaluation revealed a firm, well-defined ovoid mass. Laboratory investigations, including tumour markers (AFP, β-HCG, LDH), were within normal limits. Imaging studies demonstrated a large heteroechoic and heterogeneously enhancing solid-cystic lesion replacing the right testis, with additional metastatic deposits in the liver and lungs. The patient underwent high inguinal orchidectomy, and histopathological examination confirmed Para testicular leiomyosarcoma, characterized by intersecting fascicles of spindle cells with marked nuclear atypia and mitotic activity.
