Abstract. This study evaluates an integrated approach using the 6-minute walk test(6MWT), echocardiographic parameters, and the digital COPD Assessment Test (CAT) to early detect pulmonary hypertension (PH) in patients with chronic obstructive pulmonary disease (COPD) in primary care. A total of 60 patients participated, and high-risk individuals were identified based on walking distance, oxygen desaturation, and symptom burden measured via CAT. Results showed that patients with shorter walking distances had significantly higher PH severity, greater oxygen desaturation, and elevated CAT scores. Correlations between 6MWT, CAT scores, and pulmonary artery pressure confirmed the effectiveness of this screening approach. The integrated model enables rapid and efficient identification of high-risk patients, providing a cost-effective, scalable, and digital screening strategy, particularly useful in resource-limited settings. This approach supports early detection, monitoring, and management of PH in primary healthcare.
