Executive decision-making in regulated innovation environments demands a nuanced convergence of strategic leadership, regulatory intelligence, and organisational adaptability. This chapter examines how pharmaceutical and medical device executives navigate complex regulatory frameworks—including FDA pathways, EU MDR, and evolving AI-specific guidelines—to drive sustainable innovation. Drawing on recent empirical and conceptual literature, the chapter presents a framework integrating top management team competencies, risk governance, and digital transformation imperatives. . Key themes include the role of regulatory affairs in shaping pipeline strategy, leadership capabilities required for Industry 4.0 adoption, and the organisational enablers of innovation performance.
