PS 14-81 DETECTION OF ANXIETY AND DEPRESSION DISORDERS AT PATIENTS WITH ESSENTIAL HYPERTENSION COMPLICATED BY CHRONIC HEART FAILURE, TAKING INTO ACCOUNT SYMPATHOADRENAL SPECTRUM

Objective: This study is to detect anxiety and depression disorders (ADD) at patients with essential hypertension complicated by chronic heart failure, taking into account their sympathoadrenal spectrum. Design and Method: 40 patients of cardiological in-patient department have been surveyed. The patients had diagnosis of essential hypertension, complicated by chronic heart failure. The diagnosis was verified by anamnestic data, objective status (particular attention was paid to vegetative nervous system for detection of sympathoadrenal state), electrocardiography, veloergometry. For detection of ADD level we used Hospital Anxiety and Depression Scale (HADS) and Hamilton depression scale. Results: It was found that among the patients with chronic heart failure significant changes in sympathoadrenal system are observed at the patients with a high ADD level, on the contrary the changes among the patients with middle and low ADD level were not significant. According to HADS high ADD level was detected at 45% (18) of patients, at the same time objective status and electrocardiography showed that this group had pulse instability, variability in arterial blood pressure, liminous sensitivity, dyspnea with tachypnea, hyperhidrosis, dermographism, and in its turn that decreases the effectiveness of basis therapy and compliance, worsening general state and wellbeing of patients; middle ADD level was found at 30% (12) of patients, their objective status had changes only in arterial blood pressure within 10 grades limit during a day and tachycardia, that also decreased the effectiveness of the basis therapy; low ADD level was detected at 25% (10) of the patients, objective examination of them detected hyperhidrosis, dermographism and tachycardia. Conclusions: the research showed that the patients with chronic heart failure, affected by high level of anxiety and depression disorders, also suffer from expressed sympathoadrenal syndrome, that in aggregate brings to a decrease of wellbeing of this group of patients and to low compliance to the main somatic disease treatment.
